NOTE:  If you are ordering anything from our U.S. pharmacy, they will require your original prescription before they will fill your order, or your doctor may call or fax your prescription in.  We will provide their mailing address if you are faxing your forms.  If you are mailing your forms to us, please include the original prescriptions so we can forward them to the U.S. pharmacy.
